761.39 APPLICATION FOR LICENSE.
   (a)   Every person desiring to drive a taxicab shall submit an application for a taxicab driver's license to the Director of Finance on a form provided for that purpose. The applicant shall answer all questions in the application. An application for a taxicab driver's license shall be verified under oath and shall contain the following information:
      (1)   The names and addresses of four persons, excluding other taxicab drivers, owners and operators, who have known the applicant for a period of three years and who will attest to the sobriety, honesty and general good character of the applicant;
      (2)   The experience of the applicant in the transportation of passengers;
      (3)   The educational background of the applicant;
      (4)   A concise history of the applicant's employment;
      (5)   Disclosure of whether the applicant is currently licensed to drive a taxicab in any other locale and identify such locale; and
      (6)   Identification of all the applicant's places of residence for the past three years.
   At the time the application is filed, the applicant shall pay the Director of Finance an application fee of seventeen dollars ($17.00).
   (b)   Upon receipt of the application, the Transportation and Traffic Division shall cause to be made such investigation of the applicant as is necessary to determine whether the applicant meets the requirements set forth in this chapter. Such investigation may include a written or oral examination designed to test the applicant's knowledge of locations and landmarks within the City.
   (c)   The Police Division shall arrange with the Bureau of Criminal Identification for a background investigation of each applicant for a taxicab driver's license at the applicant's expense. A report of such investigation and a copy of the applicant's traffic conviction record, of any, obtained at the applicant's expense from the Ohio Bureau of Motor Vehicles pursuant to Ohio R.C. 4509.05, shall be attached to the application for consideration.
   (d)   When a person desiring a taxicab driver's license has submitted to the City a completed application; paid the application fee; successfully passed an examination of the applicant's knowledge of the City's locations and landmarks; provided a drug screening which does not disclose the presence of drugs being tested for; and has provided the Director of Finance with a Bureau of Criminal Identification background investigation as well as a traffic conviction record from the Ohio Bureau of Motor Vehicles; the applicant shall be granted a temporary taxicab driver's license for a period not to exceed seven calendar days provided the application forms and required attachments do not immediately reveal on their face the existence of facts or circumstances which would warrant denial of a license or the revocation or suspension of a previously issued license.
