Applications for amendments as described in section 16.13.010 of this chapter shall contain the following information:
A. Name, address, and phone number of applicant.
B. Proposed amendment and summary of the specific objective of any proposed change in text.
C. A statement of how the proposed amendment relates to the comprehensive plan.
D. A fee established in a separate ordinance approved by the city council. (Ord. 1191, 2015)
