An application for a dangerous animal permit shall be submitted in writing to the Chief of Police. The application shall include the applicant's name, address; home and business phone numbers, the address and description of the proposed location where the animal will be kept, a complete description and a color photograph of the animal, and a fee in an amount to be established by resolution of City Council.
