4-5-1: COIN OPERATED AMUSEMENT DEVICES:
   A.   License Required; Gambling Prohibited: It shall be unlawful for any person to operate, display, keep or maintain for public patronage any marble machine, pinball machine, skill ball machine, target shooting machine, mechanical grab machine and any other similar mechanical or electronic amusement machine or device which may be operated by the public generally upon the insertion or deposit of a coin, slug, token, plate or disk in any public place within the Village without first having obtained a license therefor; nor shall any such coin operated amusement machine or device be operated or permitted to be used in such a manner as to violate the gaming laws of the Village and of the State of Illinois.
   B.   Application For And Issuance Of License: Such license to operate a coin operated amusement device or devices shall be issued in accordance with the provisions of chapter 1 of this title, and in addition to the information required by section 4-1-2 of this title for application for such license, the applicant shall include on such application the applicant's telephone number and State Sales or Occupation Tax number; the names, addresses and telephone numbers of all owners, partners or officers of any proprietorship, partnership or corporation serving as applicant; the number of amusement devices to be operated by the applicant and the denomination of coin used to operate each such device; and a sworn statement that the applicant, its owners, partners or officers, have never been convicted of a felony or crime involving moral turpitude or a crime involving consumer fraud, misrepresentation or illegal and unscrupulous business activities.
