Each home occupation license issued hereunder shall state upon its face the following:
A. The name of the licensee and any other name under which home occupation is to be conducted.
B. The name, address and phone numbers of the home occupation so licensed.
C. The nature of the home occupation.
D. Whether the home occupation includes sales.
E. Home occupation business status and name, address and phone numbers of officers.
F. Site information.
G. Date of application for home occupation license.
H. Plot plan of property.
I. Floor plan.
J. The date of issuance and expiration thereof.
K. Such other information as the city clerk shall determine to be necessary. (1984 Code § 3-9-11)
