§ 153.12  PROVISIONAL PLAT REQUIRED.
   The subdivider shall provide a provisional plat of the subdivision, the design of which shall conform with the policies of the master plan and its provisions, showing the following.
   (A)   Proposed name of the subdivision, followed by the words "Provisional Plat."
   (B)   A plan including the following.
      (1)   Streets and rights-of-way, on and adjoining the site of the proposed subdivision, with names of streets which shall not duplicate names of other streets in the community except in cases of extensions of existing streets, and including roadway locations and widths, approximate gradients, curbs, sidewalks, and other pertinent data.
      (2)   The layout of lots showing dimensions and numbers; square foot area of each lot with irregular sides (not rectangular); and the block number, distances, radii, and chords.
      (3)   Parcels of land proposed to be dedicated or reserved for schools, parks, playgrounds, or other public, semipublic, or community purposes.
      (4)   Building setback or front yard lines.
      (5)   Easements:  locations, widths, and purposes.
      (6)   Names and addresses of the owner of the land, the subdivider and the city planner, land planning consultant, landscape architect, engineer, land surveyor, or other person who prepared the plan.
   (C)   Contours at vertical intervals of two feet or less if the general slope of the site is less than 10% and at vertical intervals of five feet if the general slope of the site is 10% or greater.
   (D)   Tract boundary line showing dimensions, bearings, angles, and references to section, township, and range lines or corners.
   (E)   Statement concerning the location and approximate size or capacity of utilities to be installed.
   (F)   Other features or conditions which would affect the subdivision favorably or adversely.
   (G)   Title, scale, north point, legend, notes, and date.  The provisional plat of the subdivision shall be drawn to scale of 50 feet to 1 inch, or 100 feet to 1 inch.  However, if the resulting drawing would be over 36 inches in shortest dimension, a scale as recommended by the City Plan Commission may be used.
