§ 111.13 REVOCATION OF LICENSE.
   A license issued under this section shall be revoked upon a determination that either a license is to be suspended for a second time within a 12 month period or that a licensee knowingly, or employee of a licensee intentionally and with knowledge of the licensee:
   (A)   Gave false or misleading information on the license application which enhanced the licensee's opportunity to obtain a license; or
   (B)   Knowingly allowed the possession, sale or use of a controlled substance on the premises; or
   (C)   Knowingly operated the business while under license suspension; or
   (D)   Has been convicted any offense included in § 111.07 (B) (1); or
   (E)   Knowingly operated the business in violation of the additional operating conditions of § 111.16
