Any person who possesses a mechanical or electrical amusement device shall make application for a license therefor with the Secretary/Treasurer, in writing, setting forth:
(a) His or her present and previous occupations;
(b) The length and place of his or her present and previous residences;
(c) The name and address of the owner of the premises where the devices are to be used;
(d) If the owner of the premises is not the applicant, the length of the term of the lease of the premises;
(e) If the possessor is not the owner of the device, the name and address of the owner of the said device;
(f) The location where the device is to be displayed or operated and the business conducted at that place; and
(g) The type and serial number of the device sought to be licensed.
