(a) All applications for a permit required by Section 1040.06 shall first be approved by the Manager and shall specify the name of the owner, the location of the connection and the purpose for which it is to be used. All permits shall be taken out in the name of the owner or his agent and in no case shall the work be prosecuted under the permit unless the permit is on the construction site and in the possession of the person doing the work so authorized.

(b) All applications for connection permits shall be made and issued on blank forms provided by the City for this purpose. The fee for such a permit shall be twenty-five dollars ($25.00).
