   812.03   LICENSE APPLICATION.
   Every applicant for a license required by Section 812.02 shall file with the Manager a written application stating his full name and address, including street and number, or, if more than one person, an association or a firm, the full names and addresses of all interested parties. If the applicant is a club, society or corporation, the application shall contain a complete list of the officers of such club, society or corporation; their full names and addresses; the state in which such club, society or corporation is organized; and the full names and addresses of one or more persons whom the club, society or corporation desires to designate as its manager or person in charge. The application shall also state the following:
   (a)   The location of the bowling alley, including the street and number;
   (b)   The age of the applicants, in the case of individuals, and the age of the manager and officers, in the case of a club, society or corporation;
   (c)   Whether the applicant has ever been engaged in operating a bowling alley and when, where and how long each place was operated by him within the past five years; and (d) The full name and address of the owner of the premises on which the bowling alley is located.
   The application shall be signed by the applicant or, in the case of a club, society or corporation, by the manager or any of its officers.
   812.04   INVESTIGATION BY MANAGER; REJECTION AND APPEAL.
   The Manager shall cause an investigation to be made as to the character of the applicant or the officers of the club, society or corporation and of the person who is to have the general management of the business. The application shall be rejected if the Manager finds that any person named in the application is not of good moral character or has previously been connected with any bowling alley where the license has been revoked or where any of the provisions of this chapter have been violated; or that the bowling alley for which a license is sought does not comply in every way with the regulations, ordinances and laws applicable thereto. If an application is rejected, the applicant for the license shall be notified in writing of the reasons for rejection and shall have the right to appeal to Council, which shall have the power, after a full hearing, to confirm the rejection or to order the license issued.
   In case of appeal the applicant shall, within ten days after receiving notice of rejection, perfect his appeal by leaving notice in writing of his intention to appeal at the office of the Manager.
