   812.01   BOWLING DEFINED.
   “Bowling,” as used in this chapter, means any part of the several games played on a bowling alley, by whatever name known. "Bowling alley" means any public room or place where the game of bowling is permitted to be played.

   812.02   LICENSE REQUIRED.
   No person shall open, conduct, maintain or operate a bowling alley within the City unless such person has first obtained a license for such purpose as hereinafter provided.

   812.03   LICENSE APPLICATION.
   Every applicant for a license required by Section 812.02 shall file with the Manager a written application stating his full name and address, including street and number, or, if more than one person, an association or a firm, the full names and addresses of all interested parties. If the applicant is a club, society or corporation, the application shall contain a complete list of the officers of such club, society or corporation; their full names and addresses; the state in which such club, society or corporation is organized; and the full names and addresses of one or more persons whom the club, society or corporation desires to designate as its manager or person in charge. The application shall also state the following:
   (a)   The location of the bowling alley, including the street and number;
   (b)   The age of the applicants, in the case of individuals, and the age of the manager and officers, in the case of a club, society or corporation;
   (c)   Whether the applicant has ever been engaged in operating a bowling alley and when, where and how long each place was operated by him within the past five years; and (d) The full name and address of the owner of the premises on which the bowling alley is located.
   The application shall be signed by the applicant or, in the case of a club, society or corporation, by the manager or any of its officers.
