155.38 INSURANCES.
   (a)   The employer shall provide health care insurance coverage for all full-time employees. Effective January 1, 2022, the City will pay 87%, and the employees will pay 13%, through automatic payroll deduction. Effective January 1, 2024, the City will pay 86%, and the employees will pay 14%, through automatic payroll deduction. Effective January 1, 2025, the City will pay 85%, and the employees will pay 15%, through automatic payroll deduction. Employees will not be reimbursed by the City for co-pays or deductibles with the exception that should the City contract to provide healthcare coverage through a high deductible healthcare plan which includes a health savings account obligation for the employee, the City shall contribute to said employee's health savings account 90% of the in network deductible obligation be it single, family or other. The City will reimburse a covered employee and covered family members up to seventy-five dollars ($75.00) a year for eye exam, glasses and or contacts that was paid out of pocket, after insurance payments.

   (b)   The Employer shall determine the level of benefits best meeting the needs of the employer and shall be able to change insurance carriers or self-insure.
   (c)   Any non-union employee who obtains health care coverage through another source may, with presentation of proof of such coverage, receive compensation as follows:
      (1)   Two hundred dollars ($200.00) per month for employees eligible for the family plan;
      (2)   One hundred fifty dollars ($150.00) per month for employees eligible for employee and spouse, or employee and children plan;
      (3)   One hundred dollars ($100.00) per month for employees eligible for the single plan.

   (d)   Payments will be made in the month following the month in which no health care coverage was provided by the City.
   (e)   Council members shall not be entitled to receive health care insurance through the City of Eastlake.
