155.37 FAMILY MEDICAL LEAVE.
   Eligible employees must have been employees with the City for at least twelve months and have 1,250 hours of service with the City during the twelve-month period prior to any leave. An eligible employee is entitled to take a leave without pay for up to twelve weeks in any twelve-month period because of:
   (a)   The birth of a child of the employee and in order to care for the child. The eligibility for this leave expires twelve months after the birth of the child.
   (b)   The placement of a child with the employee for adoption or foster care. The eligibility for this leave expires twelve months after the placement of the child.
   (c)   The care for a child, spouse or parent of the employee who has a serious health condition.
   (d)   A serious health condition that makes the employee unable to perform the functions of the employee's position.
   While the leave is without pay, the City will pay health insurance benefits in the same manner as if the employee were working during the twelve-week period. If applicable arrangements will need to be made for employees to pay their share of health insurance premiums while on leave.
   Upon return from leave, the employee must be returned to the same or equivalent position.
   An employee is required to use available paid vacation, sick leave or personal leave to substitute for all or part of this twelve-week leave period. Under specified circumstances, this leave may be taken on an intermittent basis. Certification of need for leave may be required by the City.
