1347.04 APPLICATION PROCEDURE FOR ERECTION PERMIT.
   Application for erection permit shall be made upon the blanks provided by the Chief Building Official and shall contain or have attached thereto the following information:
   (a)   The name, address and telephone number of the tenant and the name, address and telephone number of the property owner.
   (b)   The location of the building, structure or lot to which or upon which the sign or other advertising structure as defined in this chapter is to be attached or directed.
   (c)   The position of the sign or advertising structure as defined in this chapter in relation to nearby buildings or structures.
   (d)   Two blueprints or ink drawings of the plans and specifications, the methods of construction, attachment to the buildings or in the ground.
   (e)   If required by the Chief Building Official, a copy of stress sheets and calculations showing that the structure is designed for dead load and wind pressure in any direction in the amount required by this chapter and all other laws and ordinances of the City.
   (f)   The name of the person, firm, corporation or association erecting the sign or other advertising structure as defined in this chapter as to be erected. In lieu of written consent, a copy of the lease clearly authorizing such erection shall be acceptable.
   (g)   An electrical permit required and issued for such sign or other advertising structure as defined in this chapter.
   (h)   Insurance policies or bond as required by the City.
   (i)   Such other information as the Chief Building Official shall require to show full compliance with this chapter and all other ordinances of the community.
