11-4-3: CONTENTS OF APPLICATION:
All applications required by the terms of this title shall be completed on forms provided by the city and submitted to the city clerk, along with the required fees. The application shall include:
   A.   Proof of ownership. A current deed and title report not less than six (6) months old from a title insurance company authorized to do business in the state, showing ownership of the subject land.
   B.   Ownership list: The names, email and mailing addresses and telephone numbers of all person, firms and corporations holding interests in said land.
   C.   Consent: Signatures or written consents of the property owners and any lienholders, as shown on the current title report, authorizing the filing of the application.
   D.   Property Description: Legal description and tax parcel number of the subject land.
   E.   List of Property Owners: A list, prepared by a title company licensed to do business in the state, of the names and mailing addresses of all property owners owning property within three hundred feet (300') from the external boundaries of the entire proposal area, as shown on the current county tax roll assessment.
   F.   Utility and Access Information: Documentation of the proposed sewer, water, gas, electricity, fire protection, and transportation accesses proposed for the project and details regarding the capacity of affected agencies to serve the project.
   G.   Vicinity Map: A map of the area surrounding the subject land, showing existing road patterns, trails, and adjoining properties.
   H.   Stormwater Plan: A stormwater, erosion control plan, where applicable, in conformance with adopted city stormwater regulations.
   I.   Preliminary design or construction plans, pursuant to the requirements of this title. Depending upon the site, proposed typical sections, topography, and preliminary information such as road profiles, paving limits, intersection geometry, excavation and embankment limits to show the interaction of the new or existing stormwater features, wetlands, floodplain and floodway boundaries, waterways, seasonal tributaries and drainages, proposed and existing rights-of-way, and proposed and existing easements, where applicable, for the purpose of reviewing the reasonableness of the preliminary plat. Existing easements and record instrument numbers shall be shown on the plans.
   J.   Proposed utility infrastructure plan, including sewer master plan, water lines, fire lines and irrigation service line locations and centerline grades.
   K.   Pedestrian circulation plans.
   L.   Right-of-way landscape plan and details.
   M.   A Trip Generation and Distribution Letter prepared by an Idaho-licensed engineer. All calculations shall be based upon the latest edition of the Institute of Transportation Engineer's Trip Generation Manual. The city will accept letters prepared in accordance with the guidelines outlined in the Independent Highway District (IHD) Standards. Based upon the Trip Generation Letter, a Traffic Impact Study may be warranted, as determined by the city engineer, following guidelines from the ITE Transportation Impact Analysis for Site Development and IHD.
