4-12-4: PERMITS:
   (A)   Applications: Applications for permits to construct, repair or modify any water well shall be made to the building official of the city and shall include the following:
      1.   A plot plan indicating the exact location of the well with respect to the following items within a radius of two hundred feet (200') of the well:
         (a)   Public water lines.
         (b)   Public sewer lines.
         (c)   Property lines.
         (d)   Existing wells.
         (e)   Septic systems.
         (f)   Existing or proposed buildings.
         (g)   Access roads or streets.
      2.   Location of the property.
      3.   Owner of the property.
      4.   Name of the person who will construct, repair and/or modify the well.
      5.   Depth of the well.
      6.   Use of the well.
      7.   Other information as may be necessary to determine if underground waters will be protected.
   (B)   Fees:
      1.   Permit Application Fee:
         (a)   Every permit application shall be accompanied by a nonrefundable fee of two hundred fifty dollars ($250.00), none of which shall be refundable.
      2.   Permit Fee:
         (a)   Same fee schedule as listed in subsection 7-1-9(B)3 of this code. (Ord. 2006, 8-4-1997)
         (b)   Prior to issuance of a permit the person applying for a permit shall appear before the Dixon city council for a review of the appropriate ordinances of the city concerning public water supplies and for a review of the information provided in the application for permit.
