1313.09 APPLICATIONS FOR REGISTRATION.
   Applications for registration as a Contractor shall be made on forms prescribed by the Building and Zoning Commissioner. Said forms shall require disclosure of:
   (a)    The name, address, telephone number and tax identification number of the applicant;
   (b)    If the applicant is a corporation, the names, business address and business telephone number of all officers;
   (c)    If the applicant is a partnership or other unincorporated business association, the names, business address and business telephone number of all general partners or other principals;
   (d)    The name of all individuals employed by the applicant who are authorized to obtain permits in the name of the applicant;
   (e)    Satisfactory evidence that the applicant for registration has filed all documents ordinarily and customarily required in connection with the administration of the municipal Income Tax Ordinance with the City Finance Department, Income Tax Division;
   (f)    Satisfactory evidence of insurance coverage required by Section 1313.10 of this chapter;
   (g)    Satisfactory evidence of qualifications fulfilling the minimum requirements of Section 1313.11 of this chapter.
