(a) An application for a chauffeur license shall be filed with the Village Manager or his designate on the form provided.
(b) Such application shall include the following information:
(1) Applicant's full name, residence address, and date of birth.
(2) Applicant's employment history and experience in the transportation of passengers for hire.
(3) Current status of applicant's Illinois driver's license and number as given by the Illinois Secretary of State.
(4) Whether the applicant's State driver's license has ever been suspended or revoked and if so, when and for what cause.
(5) A list of all convictions, if any, for violation of Federal, State or municipal laws.
(6) Two personal references, unrelated to the applicant. (Ord. 0-91-55, 12-16-91)
