(a) The application for a discharge permit shall be in writing, signed by the applicant and filed with the village engineer.
(b) The application shall state the:
(1) Name and address of the applicant;
(2) Type of wastes or sewage to be discharged;
(3) Quantity to be discharged, maximum amounts per hour per day;
(4) Treatment given to discharge, if any;
(5) Proof by affidavit of compliance with all village, county, state and federal laws concerning pollution of waterways. (1963 Code)
