Sec. 6-4. Applications And Certifications; Disclosure Of Owners:
   (a)   Applications for building permits shall be submitted in duplicate to the Director of Building and Zoning and shall be accompanied by plans, elevations, critical sections and details drawn to scale and specifications so as to clearly show the method of construction and materials to be used. Applications shall also be accompanied by a written disclosure identifying the building permit applicant and, if different, the legal and beneficial owners of the zoning lot for which a building permit is requested. A duly authorized owner, trustee, corporate officer, general partner or managing agent shall make the disclosure(s), under oath, as the case may be, identifying:
      (1)   Land Trusts: The name, address and interest of all persons, firms or other entities who are the beneficiaries of such land trust.
      (2)   Corporations: The name and address of all corporate officers; the name and address of all persons who own five percent (5%) or more of any class of stock in the corporation; and the name and address of the registered agent.
      (3)   Partners And Limited Liability Corporations: The name and address of all general and limited partners.
      (4)   Limited Liability Companies: The name and address of all members and managers and of the registered agent.
      (5)   Other Persons Or Entities: The name and address of every person having a proprietary interest, an interest in profit and losses, or the right to control any entity or venture so disclosed. (Ord. 0-07-08, 2-20-2007)
   (b)   In the case of nonresidential structures, applications shall be accompanied also by plans for all signs to be constructed and installed in connection with such structures disclosing the design, size and location thereof.
   (c)   Plans, except those for single-family residences under two thousand (2,000) square feet, erected by and for the occupancy of the owner, shall be certified by a registered architect and/or engineer acting for the owner, as required by the Registration Act of Illinois. Additional necessary information shall be submitted at the request of the Director of Building and Zoning.
   (d)   Such certificate shall be in the following form:
      Architect's Certification.
      I do hereby certify that these drawings have been prepared under my direct supervision and to the best of my knowledge conform to all codes and ordinances of the Village of Deerfield, Illinois.

   (e)   No such certificate shall be required for remodeling of single-family residences done by the owner not affecting bearing walls or supports nor for exterior steps, walls, patios or accessory buildings or porches.
   (f)   During construction the architect or engineer in charge shall, at the times indicated, submit a certificate substantially as follows:
   The ordinance pertaining to inspection or supervision to be amended to include the following certification to be signed and sealed by the architect and/or engineer who has certified the drawings for any construction project:

      I do hereby certify that I myself, or an authorized representative of my firm has inspected the subject project at the following stages and find the construction in strict accordance with approved permit drawings and specifications, and any deviations therefrom as approved by the Village of Deerfield on file with the Director of Building and Zoning:
