§ 115.04 TRANSIENT MERCHANT, ITINERANT VENDOR AND SOLICITORS LICENSE.
   (A)   Any person who desires to obtain a license as a transient merchant, solicitor or itinerant vendor in the city shall file a written license application with the city clerk. The license application shall include: the applicant’s name, date of birth and last four of social security number; permanent business address (which must include a street address); residence address of the principal (and, if the applicant is a corporation, the residence address of each of its officers); applicant’s phone number; the places where the applicant has resided for the preceding year; the applicant’s occupation and employment during the preceding year; the location at which the applicant intends to do business; the nature of the business the applicant intends to conduct; a copy of the applicant’s certificate of registration under the Retailers’ Occupation Tax Act; a complete inventory of the goods the applicant intends to offer for sale; the number, description and license plate numbers, if applicable, of any vehicles or other means of transportation of goods the applicant intends to use in the course of his business in the city; a list of the persons employed or to be employed by the applicant in the city; and a list of all licenses to conduct business as a transient merchant or an itinerant vendor obtained by the applicant in this state in the 12 months preceding the date of filing of the application. Any person applying for a license who might operate a vehicle, trailer or other means of transportation of goods for purposes of sales or solicitation shall submit, along with the application, proof of insurance as to each vehicle, trailer or other means of transportation which may be used. Any person applying for a license as a transient merchant shall submit with such application a copy of a valid lease agreement between the applicant and the lessor providing space for such applicant. The city clerk shall forward the application to the police department for investigation and approval. In addition, the city clerk shall forward a copy of the application to the zoning administration for review with respect to zoning matters.
   (B)   The applicant shall pay a license application fee of $100 per month for each calendar month or part thereof specified in the application, which fee shall be non-refundable. Any license issued to the applicant shall expire on the date set forth in the license.
   (C)   The chief of police or the designee, shall investigate the application and approve the application upon being satisfied that the information so disclosed is true and correct and that the applicant is not a prohibited person under § 115.08 or otherwise prohibited from obtaining a license under § 115.10. If all such requirements have been met, the chief of police or the designee, shall so inform the city clerk, who shall then issue a license.
   (D)   The license so issued shall state: the name of the licensee; the type of business, along with a description of the merchandise, wares or goods offered for sale; the number and description of any vehicles, trailers or other means of transportation to be used; and the dates through which the license is valid.
