1319.03 APPLICATIONS FOR REGISTRATION.
(a) Application. Every application for registration shall be in writing and shall contain the name, phone number and the place of business of the applicant. If the person making the application is representing a firm, corporation, partnership, association, or other entity, there shall be submitted on behalf of the applicant a certified copy of the Certificate of General Partnership or Certificate of Limited Partnership, a certified copy of corporate minutes or other credible evidence showing that the applicant is duly authorized to represent and act for the firm, corporation, partnership, association or entity, in all matters regulated by the City Building Code and other applicable building or construction codes of any governmental entities. The Chief Building Official may request any additional information which he believes is necessary to evaluate the application. The application shall also list the capacities in which the applicant desires to register.
The applicant also shall certify on the application that the work it undertakes shall be of good workmanlike quality and in accordance with the City Building Code or other applicable building or construction codes of any governmental entity.

(b) Insurance. No registration or renewal for registration shall be issued unless the application is accompanied by an insurance policy issued on the registrant in the following minimum amounts:
(1) Liability insurance in an amount of not less than one hundred thousand dollars ($100,000) for injuries, including those resulting in the death of any one person and in an amount of not less than five hundred thousand dollars ($500,000) on account of any one accident or occurrence.
(2) Property damage insurance in an amount of not less than one hundred thousand dollars ($100,000) for damages on account of any one accident or occurrence.
(c) Annual Registration Fee. Upon submission of any application for registration, the applicant shall pay a non-refundable annual fee of one hundred dollars ($100.00). The applicant shall receive from the Chief Building Official a certificate of registration.
