Applicants for a permit under this chapter must file directly with the City Council through its agent, the City Manager, on a form designated, prescribed and furnished by the Commission. The sworn, written application must contain the following:
(A) Name(s) of individual(s) applying and a permanent mailing address where the individual(s) may be reached;
(B) A brief description of the expected size of the fair/carnival, including what attractions will be contained;
(C) Proof of a current N.D.T. X-Ray inspection;
(D) Proof of liability insurance in the minimum amount of $1,000,000 per incident; and
(E) A $10 non-refundable application processing fee.
