   (B)   No water service connection to any premises shall be installed or maintained by the water system unless the water supply system is protected as required by state laws and this chapter. Service of water to any premises shall be discontinued by the utility system if a backflow prevention device required by this chapter is not installed, tested and/or maintained; or if it is found that a backflow prevention device has been removed, bypassed or if an unprotected cross connection exists on the premises.    (D)   If, in the judgment of the manager or his or her designated agent, an approved backflow prevention device is required at the water service connection to a customer's premises, or at any point(s) within the premises, to protect the potable water supply, the manager shall compel the installation, testing and maintenance of the required backflow prevention device(s) at the customer's expense.
   (E)   An approved backflow prevention assembly shall be installed on each water service line to a customer's premises at or near the property line or immediately inside the building being served; but in all cases, before the first branch line leading off the service line.

All required assemblies shall be installed and operational prior to the initiation of water service.
   (G)   For existing premises, personnel from the water system shall conduct inspections and evaluations and shall require correction of violations in accordance with the provisions of this chapter.
   (H)   For existing installations, the Cross Connection Manager/Coordinator may cause water service to be discontinued until the time as the customer complies with all requirements of state law and this chapter.
