(A) Permits issued under this chapter shall bear the name and address of the person by whom the solicitation is to be made, the number of the permit, the date issued, the dates within which the permit holder may solicit and a statement that the permit does not constitute an endorsement by the city or by any of its departments, officers or employees of the purpose or of the person conducting the solicitation. All permits shall be signed by the Chairperson of the Board and the Secretary thereof or by their duly authorized officer or agent. Permits may be granted for a period of 90 days or for other or additional periods as the Board determines to be proper, but in no event shall the period for which the organization is authorized to solicit exceed one year. The form of the permit or certificate shall be as follows:

(B) (1) Any permit issued hereunder shall be non-transferable and the fact of non-transferability shall be clearly indicated on the permit.
(2) Each permit issued under this chapter shall be returned to the Board within 72 hours of the date of expiration, together with all facsimile copies thereof.
