8-1-2: FEES, CHARGES, AND GENERAL PERMIT REQUIREMENTS:
   A.   Permit Inspection Fees For New Construction; Structures Regulated By The International Residential Code:
      1.   The permit fee inspection shall be one dollar ($1.00) for each square foot of floor area.
      2.   The "floor area" of a building or buildings shall be the sum of the gross horizontal areas of the several floors of such building or buildings measured from the exterior faces of exterior walls or from the centerline of party walls separating two (2) buildings. In particular "floor area" shall include:
         a.   Basement space meeting any of the following conditions:
            (1)   At least one-half (1/2) of the basement story height is above the established curb or ground level;
            (2)   The basement story contains habitable space as defined by the adopted international residential code;
         b.   Elevator shafts and stairwells at each floor;
         c.   Floor space used for mechanical equipment where the structural headroom exceeds seven and one-half feet (71/2'); except equipment, open or enclosed, located on the roof, i.e., bulkheads, water tanks and cooling towers;
         d.   Attic floor space where the structural headroom exceeds seven and one-half feet (71/2');
         e.   Interior balconies and mezzanines;
         f.   Enclosed, or covered porches, but not terraces and breezeways;
         g.   An accessory building when attached to the principal building.
      3.   There shall be an electrical and zoning plan review fee of one hundred and fifty dollars ($150.00).
      4.   The building, plumbing and engineering reviews are performed by an outside consultant. Those fees are payable to the city per subsections E and F of this section.
      5.   The building official shall approve and issue all permits.
      6.   Inspections shall be as required in the adopted international residential code.
   B.   Permit Inspection Fees For Remodeling And Alteration; Structures Regulated By The International Residential Code Including Additions, Remodeling Projects And Construction Of Accessory Structures:
      1.   There shall be a minimum fee of fifty dollars ($50.00) base fee plus twenty five dollars ($25.00) for each discipline (electrical, plumbing, HVAC) for projects less than 250 square feet.
      2.   For projects greater than 250 square feet, the permit inspection fee shall be thirty cents ($0.30) per square foot of floor area plus fifteen cents ($0.15) per square foot of floor area for each electrical, plumbing and mechanical included in the project.
      3.   The "floor area" shall be the gross square foot area of all rooms or areas affected during the construction project.
      4.   In the event weather related or emergency conditions necessitate application for a building or demolition permit, the building official may, in his discretion and for a reasonable period of time following such event, suspend the building or demolition permit fees specified herein where it appears to him that such weather related damage or emergency condition has caused or contributed to property related damage and the repair of such damage is the subject of an application for a building or demolition permit.
      5.   Inspections shall be as required in the adopted international residential code.
      6.   The building official shall approve and issue all permits.
   C.   Permit Inspection Fees For Demolition; Structures Regulated By The International Residential Code:
      1.   There shall be a minimum fee of twenty five dollars ($25.00).
      2.   The permit fee shall be fifty cents ($0.50) for each square foot of footprint of the structure to be demolished.
      3.   The building official shall approve and issue all permits.
   D.   Permit Inspection Fees For All Construction, Renovation, Or Demolition Regulated By The International Building Code:
      1.   There shall be a minimum fee of fifty dollars ($50.00).
      2.   The permit fee shall be twenty dollars ($20.00) per each one thousand dollars ($1,000.00), or portion thereof, of the established valuation of cost of the proposed work. Prior to the issuance of the certificate of occupancy, the applicant shall submit to the building official a sworn and notarized statement compiling all construction costs and expenses, or copies of all appropriate lien waivers. The permit fee shall be adjusted to reflect the final valuation of the work. This number shall not be less than as determined in subsections D3 and D4 of this section.
      3.   The valuation shall be determined on the estimated cost of construction per square foot unit thereof; said estimated cost to be commensurate with current costs per square foot unit of construction for similar type structure in the suburban area of Cook County. Estimated cost shall not be based upon actual cost of construction when, due to such factors as availability of labor and material at less than current costs for similar structures in Cook County, actual cost is less than such current cost.
      4.   The city council or its authorized agent shall establish, upon investigation of the site of construction and the examination of plans and specifications for any such proposed improvement, the value per square foot and the number of square feet involved.
      5.   Each applicant for a commercial building permit or office building permit shall be required to construct a concrete sidewalk to a width of not less than five feet (5') and not less than four inches (4") thick (not less than 6 inches thick at any place where sidewalk passes over a driveway), the entire width and/or length of any side of the property for which the commercial building permit or office building permit is issued which faces upon a street. Any such sidewalk shall meet the following minimum construction requirements: Concrete shall be of six (6) bags per cubic yard mix with a minimum strength of four thousand (4,000) psi, shall be poured over a base of not less than two inches (2") of crushed stone, and shall be sealed after pouring. However, any requirement hereof may be waived or modified by the building official when he finds that construction of such a sidewalk or a part thereof will provide no public benefit or such public benefit is outweighed by a hardship which will result to the applicant if such sidewalk is constructed.
      6.   The building official shall approve and issue all permits.
   E.   Reimbursement To City For Engineering Charges: In addition to any other charges or fees, the applicant for any building permit which requires the city engineer's examination of plans, a topographical survey, or any other survey or document, shall reimburse or pay to the city for the cost of such engineering examination before the permit applied for shall be issued.
   F.   Reimbursement To City For Other Charges: The applicant for any building permit shall reimburse or pay to the city for the cost of all expenses incurred by the city of Countryside in the preparation, review, and issuance and maintenance of such permit. These costs include, but are not limited to, the required job site construction sign, any emergency measures for the protection of the public, and/or cleaning of the public way, if the permit holder does not do so on a daily basis. Such costs shall be fully reimbursed prior to any of the following actions:
      1.   Permit issuance.
      2.   Temporary certificate of occupancy.
      3.   Certificate of occupancy.
      4.   Refund of completion bond, or any other funds.
      5.   Permit plan review and administration fees for permit applications with no permit issuance beyond one hundred and eighty (180) days and are therefore considered expired.
   G.   All Construction; General Permit Requirements:
      1.   These requirements are in addition to and supplemental to all requirements pursuant to the building code for the city of Countryside as it is from time to time amended.
      2.   All building permits, whether for residential or commercial, must include the name, address and telephone number of the general contractor and all subcontractors.
      3.   All building permits must include at least one 24-hour telephone number.
      4.   All building permit applicants must execute an acknowledgment provided by the city that the applicant has read, understands and agrees to all the terms and conditions set forth in the building permit package as well as the city of Countryside building code.
      5.   A temporary construction sign shall be placed at all residential and commercial construction sites which shall include the name of the general contractor, telephone number for the general contractor, permit information and general construction rules established by the city of Countryside. The permit holder shall purchase such sign from the city of Countryside.
      6.   All permit holders are responsible for the cleaning of the street within two hundred fifty feet (250') of the work site every day. (Ord. 10-61-0, 12-8-2010)
   H.   Fees And Fines For Violations Of The City Building Codes:
      1.   Work Started Without A Permit: For any and all work which is started without first obtaining the proper or required permits, the following fees shall be assessed:
         a.   One- and two-family residential properties shall be assessed a fine (in addition to the regular permit fee) of an amount equal to two (2) times the permit fee or one hundred fifty dollars ($150.00) whichever is greater.
         b.   Commercial, industrial and multi-family properties shall be assessed a fine (in addition to the regular permit fee) of an amount equal to two (2) times the permit fee or two hundred fifty dollars ($250.00) whichever is greater.
         c.   Any person or company in violation of this section two (2) or more times shall be issued a citation to appear before the city of Countryside adjudications where a fine up to seven hundred fifty dollars ($750.00) per day may be ordered.
      2.   Failed Inspections/Re-Inspection Fees: A seventy five dollar ($75.00) reinspection fee shall be assessed for each failed inspection on work covered under the scope of an issued permit. The reinspection fee must be paid prior to scheduling the next inspection.
      3.   Work Not Ready For Inspection: An additional one hundred fifty dollar ($150.00) fee shall be assessed for each scheduled inspection for which the work is not complete or access is not provided to the job site by the contractor, owner or agent.
      4.   Improper Display Of Permit Placard: A fifty dollar ($50.00) fine shall be assessed for any work being completed without properly displaying the permit placard. The permit placard shall be displayed on the front of the building and be clearly visible from the public way. A warning of one non-compliance event will be issued in writing prior to any fines being issued.
   I.   Plan Review Fees For Proposed 1 & 2 Family Detached Single Family Residences, Attached Single Family Residences, Residential Additions And Residential Accessory Structures As Regulated By The International Residential Code (IRC):
      1.   New Single-Family Residences And Attached Single Family Including Additions:
         a.   Less than 250 square feet: one hundred dollars ($100.00) plus fifty dollars ($50.00) per trade review for plumbing and electrical.
         b.   250 square feet to 749 square feet: two hundred dollars ($200.00) plus fifty dollars ($50.00) per trade review for plumbing and electrical.
         c.   750 square feet to 1,999 square feet: four hundred dollars ($400.00) plus one hundred dollars ($100.00) per trade review.
         d.   2,000 square feet to 4,999 square feet: five hundred dollars ($500.00) plus one hundred twenty-five dollars ($125.00) per trade review.
         e.   5,000 square feet or greater: seven hundred dollars ($700.00) plus one hundred seventy-five dollars ($175.00) per trade review.
         f.   Clerical/administration review: fifty dollars ($50.00).
      2.   Accessory Structures Plan Review:
         a.   Detached Garages: one hundred fifty dollars ($150.00).
         b.   Swimming Pools: two hundred fifty dollars ($250.00).
         c.   Fences: seventy-five dollars ($75.00).
         d.   Furnaces, Air Conditioners, Siding, Roofs, Sheds: seventy-five dollars ($75.00).
         e.   On-Grade Patios: seventy-five dollars ($75.00).
         f.   Decks: one hundred fifty dollars ($150.00).
         g.   Clerical/administration review: fifty dollars ($50.00).
      3.   Commercial, Industrial And Multi-Family Plan Reviews As Regulated By The International Building Code:
         a.   Plan Review Fee: construction value x .004.
         b.   Minimum Plan Review Fee: four hundred dollars ($400.00) for the first one hundred thousand dollars ($100,000.00) of construction value plus 25% of plan review fee per trade review.
         c.   Clerical/Administrative Review: one hundred dollars ($100.00).
         d.   NFPA 13 Sprinkler Reviews: outside consultant fee reimbursement.
         e.   Commercial Kitchen Hoods: two hundred fifty dollars ($250.00).
