3-4-6-4: APPLICATION FOR LICENSE:
An application for a restaurant license shall be made in writing to the city clerk in such form as he shall require. Each application shall state the name of the applicant, location of the premises and shall contain such further information as may be required by the health officer in order to inform him fully as to the size and nature of the place to be used for the purposes of the business and the conditions, equipment and facilities for conducting the business therein, the health of persons to be employed, and the regulations and facilities provided for them.
The city clerk shall issue a restaurant license to such person who complies with the requirements of this section 3-4-6; provided, however, that no such license shall be issued by the city clerk unless the health officer has certified that he has completed his inspection as required under the provisions of section 3-4-8-4 of this chapter, and that the applicant has complied with all applicable provisions of this section 3-4-6 relating to health and sanitation; and provided further, that no such license shall be issued by the city clerk unless the building commissioner has certified that he has completed the inspection required to be made by him under the provisions of section 3-4-8-4 of this chapter and that the applicant has complied with all provisions of the building code 1 .
