3-1-2: APPLICATION FOR LICENSE:
   A.   Before any license required by this code shall be issued, application shall be made to the city clerk on forms provided by the clerk. Each shall be completed and executed by the business to be licensed through its owner or its duly authorized agent and shall state:
      1.   The name under which the business is to be carried on. (1970 Code §8-0.10)
      2.   The following information as to owners, officers and agents:
         a.   In the case of a sole proprietorship, the name, address and phone number of the owner.
         b.   In the case of a partnership, the name, address and phone number of each partner.
         c.   In the case of a corporation, the name, address and phone number of the president, secretary, treasurer and registered agent.
         d.   In the event the business to be licensed has no owner, partner or officer residing within the city, or no registered agent residing or having a permanent office within the city, then the applicant shall also provide the name, address, and phone number, within the city of some person authorized to receive notices and legal process, including summonses, on behalf of the business, who resides within the city or whose duty it will be to be upon the licensed premises during normal business hours.
         e.   Any change in the name, address or phone number of an owner, partner, corporate officer, registered agent, or person authorized to receive notices and legal process, shall be reported to the office of the city clerk within seven (7) days of such change; failure to report such change within the time specified shall be grounds for revoking or suspending a business license.       3.   The type of business to be carried on and its location.
      4.   That the owner, owners or corporate officers are of good moral character and have never been convicted of a crime.
      5.   The type of license desired.
      6.   The time period for which the license is requested.
      7.   Such further information as may be required by this code, any other applicable ordinances or as may be needed for proper guidance of the city officials issuing the license. (1970 Code §8-0.10)
   B.   No business license shall be issued until all information requested by the city clerk or other city officials has been supplied, and all other provisions of this code or other applicable city ordinances have been complied with.
   No license shall be issued until the new business has applied for and passed a new business building department inspection to include a building inspection, electrical inspection, fire department inspection and health department approval (if necessary).
   Any existing violation of this code or other city ordinance by the applicant/business at the time of application for a business license shall be grounds for the refusal to issue a business license.
   No business license shall be issued until all outstanding fees and/or fines owed to the City of Countryside are paid in full to the Clerk's office.
   C.   Each application must be signed and verified under oath by the owner or duly authorized agent of the business to be licensed. When application is by agent, the city clerk shall not issue the license applied for until he receives satisfactory evidence of the agent's authority. (1970 Code § 8-0.10; amd.
