959.05 LICENSING OF CONTRACTORS.
   (a)   No person shall engage in garbage collection services without first obtaining an annual license from the City Manager, said license to run from September 1 to August 31st each year. Those haulers currently licensed by the Board of Health shall be deemed registered until August 31, 2017.
   (b)   Those persons seeking a permit to engage in garbage collection shall submit an application to the City Manager's Office accompanied by a licensing fee of $1 ,500.00.
   (c)   An application for a hauling permit shall include the following information:
      (1)   The name, address and phone number of the company, the name, address, phone number and email address of the company representative, and the company EIN.
      (2)   The name and address of the landfill to which trash, garbage, and refuse are taken and unloaded.
      (3)   The name and address of the recycling center to which glass, plastic, paper, aluminum and other recyclable materials are taken and unloaded.
      (4)   Verification that its employees are covered by Workers Compensation, and that the contractor has liability insurance of at least $1 million dollars for the vehicles used in its hauling operation.
      (5)   Verification that the contractor has a recycling program that meets the conditions of this chapter. (Ord. 110-16. Passed 2-13-17.)
      (6)    Certification that each vehicle used has passed a safety inspection test by the Ohio State Patrol or other certified mechanic approved by the City within 60 days of filing its application.

   (d)   An application shall be approved once it has been determined that the applicant has satisfied all the criteria in Section 959.05(c). (Ord. 110-16. Passed 2-13-17.)
