(A) An application for a vehicle for hire permit shall be filed with the City Clerk upon forms provided by the city. The application shall be verified under oath and shall contain the following information:
(1) The name and address of the applicant. If the applicant is a corporation, the name and address of all stockholders, directors, officers of the corporation, along with the principal place of business of the corporation. If the applicant is a partnership, names and addresses of all general and limited partners of the partnership, along with the address of the principal place of business of the partnership;
(2) The financial statement or balance sheet of the applicant. A listing of any unpaid judgements pending either against the applicant or any person or legal entity owning any interest in the applicant, including a brief description of the nature of the litigation or proceedings out of which any such judgement arose;
(3) Proof of financial responsibility as defined in § 114.17 of this chapter;
(4) Any facts which the applicant believes tend to prove that public convenience and necessity require the granting of a vehicle for hire permit;
(5) The number of vehicles to be operated or controlled by the applicant/business partnership and corporation. The application shall include the make, model, VIN number, state license plate number, and insurance policy number of each taxi or vehicle for hire;
(6) A vehicle inspection report for each vehicle the applicant intends to put into operation under its permit that complies with the requirements of § 114.13 of this chapter. The fee for each vehicle shall be the same as set forth in § 114.04 of this chapter and the fee shall accompany the application;
(7) The insignia to be used to designate the vehicle or vehicles of the applicant;
(8) The proposed rates of fare; and
(9) Whether the applicant has ever had a similar license suspended or revoked and if so the details thereof.
(B) Pursuant to Iowa Code § 321.241(2), a vehicle for hire company applying for a permit is not required to maintain a physical place of business in the city as a condition of receiving a vehicle for hire permit under this chapter. All vehicle for hire business permit holders and vehicle for hire drivers shall comply with the requirements of Chapter 159 of this code regarding the storage and parking of vehicles covered by this chapter.
(C) No vehicle for hire permit shall be sold, assigned, pledged, or otherwise encumbered or transferred in any way to any other operator without the express prior approval of the Council.
(D) Upon the filing of an application, the City Clerk shall forward a copy of the same to the Chief of Police or designee to investigate the applicant, and the vehicles subject to licensing and file a written report and recommendation with the City Clerk.
