An applicant for a license under this article shall file a written application with the Director of Finance, signed by the applicant, if an individual, by one of the partners, if a partnership, or by one of the officers, if a corporation, showing:
(a) The name of the person having the management or supervision of the applicant’s business during the time it is proposed that it will be carried on in the City.
(b) The local address of such person while engaged in such business.
(c) The permanent address of such person.
(d) The capacity in which such person will act; that is, whether as proprietor, agent or otherwise.
(e) The name and address of the person for whose account the business will be carried on, if any.
(f) If a corporation, under the laws of what state the same is incorporated.
(g) Three letters of recommendation from reliable property owners in the County certifying as to the applicant’s good character and business responsibility, or in lieu thereof, other evidence which establishes to the satisfaction of the Director of Finance, the good character and business responsibility of such person.
(h) The place in the City where it is proposed to carry on the applicant’s business, and the length of time during which it is proposed that such business shall be conducted, and the length or term of lease, if any.
(i) A statement of the nature, character and quality of the goods, wares or merchandise to be sold or offered for sale by the applicant in the City.
(j) Whether or not the person having management or supervision of the applicant’s business has been convicted of a felony or misdemeanor crime, or violation of any municipal ordinance, the nature of such events and the punishment assessed therefor.
(k) A history of the business, if deemed necessary.
(l) Credentials from the person for which the applicant proposes to do business, authorizing the applicant to act as such representative.
(m) Such other reasonable information as to identify the character of the person having the management or supervision of the applicant’s business or the method or plan of doing such business as the Director of Finance may deem proper to fulfill the purposes of this article and the protection of the public good.
