(a) Upon complying with the provisions of Section 507.03, there shall be issued to the owner a numbered metallic tag, stamped with the number and the year for which issued. The shape or design of such dog shall be changed from year to year.
(b) Every owner is required to see that the tag is securely fastened to the dog’s choke, chain, collar, or harness which must be worn by the dog at all times.

(c) The name and address of the owner of the dog shall be securely fastened to the dog’s choke chain collar or harness which must be worn by the dog at all times.
