(a) Council hereby authorizes the City Auditor to make payments for indigent burials in accordance with Ohio R.C. 5101.521.
(b) Burial expenses for eligible persons as defined in Ohio R.C. 5101.521, shall be paid in the amount specified in subsection (c) hereof upon the City Auditor receiving an itemized sworn statement of the total funeral, cremation, cemetery, and burial charges submitted by the funeral director and a sworn indigent burial affidavit signed by the Chief of Police.
(c) The maximum amount payable to the funeral director shall not exceed one thousand five hundred dollars ($1,500.00). Such sum shall include the entire cost of burial including the graveside stone or concrete marker on which the name and age, if known, and the date of the person's death shall be inscribed.
