The Director of Public Service shall, upon application duly made on forms established by his office, grant curb cuts. The following procedures for curb cuts are hereby established:
(a) The Director of Public Service shall, upon receiving the application, refer the request to the City Engineering Department for review. The City Engineer, within two weeks after receiving such application, shall return the application with a recommendation of approval or disapproval of the request. The reason for disapproval shall be set out in the application. The Director of Public Service shall prepare the application form consistent with the provisions of this section.
(b) The maximum residential curb cut shall not exceed twenty feet.
(c) The maximum commercial curb cut shall not exceed thirty feet.
(d) Any request for a curb cut greater than those listed in subsections (b) and (c) hereof must be submitted to the Board of Zoning Appeals for their approval. The procedures set out in subsection (a) hereof shall be followed. Upon return of the application from the Engineering Department approving the application, the Director of Public Service shall contact the applicant within two weeks of receipt with instructions to make further application to the Board of Zoning Appeals.
(e) Approval for a curb cut shall be on a permit prepared by the Director of Public Service, with the endorsement of the City Engineer thereon, including all specifications for construction and a diagram of the curb cut prepared by the Engineering Department attached thereto. The permit may be a part of the application form.
(f) The fees for the curb cut permits are hereby established as follows:
(1) Residential $25.00
(2) Commercial 75.00
(3) Special (see subsection (d) hereof) 100.00
