The purchaser of an article as defined in Section 735.01(a) shall prepare and deliver or mail each day to the Police Department before noon, on forms to be furnished by the Chief of Police, a complete description of each article purchased or received during the preceding day, the date the purchase was made and a description as to name, address and social security number of the person from whom it was purchased.
