141.05 APPLICATIONS.
   (a)   Applicants. All applicants must be citizens of the United States and must have attained the age of 20 years. No person shall be eligible to receive an original appointment as a police officer before he or she has attained the age of 21 years.

   (b)   Application forms furnished. The Commission shall require persons applying for an examination for original appointment to file a completed application with the Commission, the form and content of which shall be prescribed by the Commission, within a reasonable time prior to the examination.
 
   (c)   Completion of application forms. Application blanks must be completed in full and prepared in ink or be typed. The application shall be signed by the applicant, in his or her handwriting, and filed in the office of the Commission within the deadline fixed by the Commission for the particular examination.
 
   (d)   Filing fees. A filing fee, to be established by the Civil Service Commission, shall be paid by all persons filing an application for examination to receive an original appointment to the Police Department.
 
   (e)   Rejection of application. The Commission may remove from further consideration at any time the application for original appointment of an applicant who:
      (1)   Lacks any of the minimum qualifications set forth in the official class specifications;
      (2)   Is not within the prescribed age limits;
      (3)   Has made a false statement of any material fact or practiced, or attempted to practice, deception in his or her application;
      (4)   Has been certified to a previous eligible list and subsequent1y disqualified by the Commission in accordance with § 141.07(h);
      (5)   After notification, the applicant did not promptly appear at the time and place designated for the examination;
      (6)   Was disqualified for appointment through the examination process after having been certified to the immediately preceding certified eligible list for the same classification; except that an applicant disqualified only for medical reasons may compete upon presentation of a statement from a licensed physician designated by the City Manager that the disqualifying factors have been corrected.
 
   (f)   Current employees. All persons who have been continuously employed, served their probationary period and served an additional period of not less than 90 days shall retain their positions under the applicable civil service provisions in the appropriate category and classification, without being required to take an examination for the position held at the time this ordinance is effective.
