11-1-3: HEALTH CERTIFICATES FOR FOOD HANDLERS:
   A.   Certificate Required:
      1.   It shall be unlawful for any person in the City to engage in the manufacture for sale of bread, pastry, candy, ice cream or any other article of food or handle any food that is to be served to customers in any restaurant or hotel in the City without first having procured and filed with the legally constituted local health authority of the City a certificate signed by a regularly licensed physician, who is approved by such authority, that such person has been examined by such physician within thirty (30) days and shows no evidence of any communicable disease. Such examination shall include any laboratory tests that the legally constituted local health authority may by resolution prescribe. Like certificates shall thereafter be procured and filed every three (3) months, on January 1, April 1, July 1, and October 1 of each year, based upon examinations made during the previous months.
      2.   It shall be unlawful for any new employee, owner, employer or proprietor to engage in the manufacture or handling of any food as provided in the preceding subsection without first having so procured and filed a certificate as provided in such subsection.
   B.   Forms Furnished By Local Health Authority: All such certificates shall be on forms furnished by the legally constituted local health authority and shall give the name of every person examined. (Ord. 710 § 2)
