§ 114.42 APPLICATION INFORMATION.
   Applicants for licenses must file with the Mayor a sworn application in writing on a form to be furnished by the Mayor, which form shall contain the following information:
   (A)   Name and permanent address of company for whom salesman is working.
   (B)   Name, local address, and local telephone number of person in charge of salesman seeking to be licensed.
   (C)   Name and local address of person to be licensed.
   (D)   General nature of business.
   (E)   Period of time for which license is desired.
   (F)   Number of proof-of-licensing cards requested.
