1114.03 APPLICATION PROCEDURES.
   All applications for either a Minor or Major Site Plan Review shall be submitted to the Planning and Zoning Office on forms provided by the Village of Cadiz. Each application shall be
accompanied by the payment of a fee as specified in Chapter 1127. Applicants will be required to submit the following information along with the completed application form:
   (a)    Cover Letter: A letter submitted by the applicant which describes the proposed project and what it encompasses. Such letter be of no specific form but shall be as descriptive as possible. The cover letter shall also provide a statement supported by substantiating evidence regarding the requirements enumerated in Section 1114.03 .
   (b)    Site Plan: The applicant shall submit six sets of site plans for the proposed project showing the following items:
      (1)   General Vicinity Map.
      (2)   Property Boundary Map.
      (3)   Elevation Marks and Contours.
      (4)   Traffic and Circulation Plan.
      (5)   Adjacent Streets.
      (6)   Parking and Loading Plan.
      (7)   Landscaping Plan.
      (8)   Grading and Surface Drainage Plan including Preliminary Storm Water Calculations, and description of best management practices.
      (9)   Utilities Plan (water and sewer).
      (10)   Proposed Signage.
      (11)   Any additional information required by the Planning and Zoning Office and/or the Planning Commission which is deemed necessary for the review of the application. The Planning and Zoning Office may waive one or more of the above items for projects only requiring Minor Site Plan Review. The Planning and Zoning Office will make the final determination as to the adequacy of the submission and may reject any submission which is determined to be incomplete.
   (c)   Property Owner List: The applicant shall provide the names and addresses of the property owners within 200 feet of the property lines of the project property being considered. The names and addresses shall be those that appear on the County Auditor’s current tax list or the County Treasurer’s mailing list. Alternatively, the Planning and Zoning Office may waive this requirement if the Planning and Zoning Office can provide this information.
   (d)    Certificate of Appropriateness: In cases where applicable, the applicant shall provide a Certificate of Appropriateness from the Architectural Review Board, when such a review is required. The Certificate shall be issued for a specific project and plans being submitted for Minor and Major Site Plan Reviews.
