An applicant for a permit for a child care facility shall furnish to the city the following information:
1. Full name and address of the applicant;
2. The location at which the proposed child care facility is to be operated; and
3. An affidavit from the applicant to the effect that he or she complies fully with the licensing requirements for child care centers established by the state and has a valid license issued by the State Department of Human Services, or has been approved for such license by the local licensing services unit.
