§ 52.014  PERMIT APPLICATION REQUIREMENTS.
   (A)   All users shall submit the information required by this ordinance and Ordinance No. 2014-1-SO on the User Permit application form supplied by the POTW (or attached thereto) at a level of detail and in units and terms as determined necessary by the POTW to adequately evaluate the application, accompanied by payment of a permit application review fee. A separate application and supporting documentation shall be submitted for each separate location for which a User Permit is required.
