Application for such certificate of qualification shall be made on forms provided by the Building Commissioner and shall set forth the name of the applicant, his age, his place of business, his experience in the work for which a registration is applied and such other information as the Commissioner may require in order to determine the qualifications of such applicant for the work proposed to be done under such certificate. The Commissioner, in determining whether the applicant is qualified to receive the certificate applied for, may grant such certificate upon finding that the applicant has been duly licensed by any other testing community or the State, if the Commissioner is satisfied that an adequate investigation or examination has been made by such municipality of the applicant's qualifications for the work for which such certificate was issued.
(a) The applicant shall have a minimum of three years practical experience and an affidavit to this effect shall be provided.
(b) The applicant shall provide any and all references as may be requested on the application form in the manner provided.
(c) If the applicant's work or work done under the applicant (as a general contractor) includes plumbing, electrical or gas installation, any installer working in the City of Brook Park shall have the following:
(1) A valid certificate of qualification from the City of Cleveland, (formally called Journeyman Plumbers License), or Electrician, or HVAC Fuel/Gas Installer, which ever is applicable;
(2) A valid Journeyman Plumbers, Electricians, HVAC/Gas Installer License from a municipality or county in Ohio that required a comprehensive examination, whichever is applicable;
(3) Proof of passing the Exterior "Plumbing with Gas" examination (Columbus version).
(d) Violators of this provision will be issued a notice and immediately removed from the work site. A second offense will result in a three-year ban from working on any of the plumbing, electrical or HVAC/Fuel Gas work in the City of Brook Park.
(e) Apprentices registered with the Ohio State Apprenticeship Council are exempt from this section.
