765.06 MASSAGE THERAPIST LICENSE APPLICATION.
   (a)   An application for a massage therapist license shall be submitted to the Safety Director on a form provided by the Department of Public Safety. The application may request, and the applicant shall provide, such information as reasonably necessary (including fingerprints) to enable the City to determine whether the applicant meets the qualifications established in this chapter.
   (b)   The completed application for a massage therapist license shall contain the following information and shall be accompanied by the following documents:
      (1)   The applicant's name and any other names or aliases used by the applicant.
      (2)   The applicant's date and place of birth.
      (3)   The applicant's height, weight and hair and eye color.
      (4)   A recent photograph of the applicant, taken by the Brook Park Police Department, which clearly shows the applicant's face.
      (5)   The applicant's fingerprints, recorded by the Brook Park Police Department.
      (6)   A description and identification of the location of any tattoos on the applicant's face, arms, legs, hands or any other anatomical area that normally would be visible when the applicant is on the premises of the proposed massage establishment.
      (7)   The applicant's present residence address and telephone number.
      (8)   The applicant's present or intended business address and telephone number.
      (9)   The applicant's driver's license number and Social Security number.
      (10)   Proof that the applicant is at least eighteen years old.
      (11)   A statement detailing the massage-related license history of the applicant for the five years immediately preceding the date of the filing of the application, including whether such applicant previously operated or is seeking to operate a massage establishment in this or any other jurisdiction, and whether the applicant has ever had a massage-related license, permit or authorization to do business denied, revoked or suspended. In the event of any such denial, revocation or suspension, the applicant shall state the name of the issuing or denying jurisdiction and the date of, and reason for, the denial, revocation or suspension. A copy of any order of denial, revocation or suspension shall be attached thereto.
      (12)   A statement as to whether the applicant has been convicted of a specified criminal activity, as defined in Section 765.01 of this chapter, and, if so, the specified criminal activity involved and the date, place and jurisdiction of each such conviction.
   (c)   The applicant shall undergo a physical examination for contagious and communicable diseases, which shall include at a minimum a recognized blood test for syphilis, a culture for gonorrhea, and a test or tests which will demonstrate freedom from tuberculosis, which procedures shall be made and interpreted by a licensed physician acceptable to the Health Commissioner. Such other laboratory tests shall be done, in a laboratory acceptable to the Health Commissioner, as may be necessitated by the above examination. The applicant shall also submit to the Health Commissioner a certificate, based upon the applicant's physical examination and issued within thirty days of such examination, signed by a physician duly licensed by the State and affirming that the person examined is either free from any contagious or communicable disease or incapable of communicating any such disease to others.
