721.06 LICENSE SUSPENSION OR REVOCATION; HEARINGS.
   (a)   The license of any person violating any of the terms of this chapter or any of the rules and regulations established and adopted by the Mayor as provided in Section 721.02, except those relating to the exhibition or operation of such mechanical or electrically operated amusement device for gambling, shall, for the first violation thereof, be suspended by the Mayor for not less than ten nor more than thirty days; for the second violation thereof, be suspended by the Mayor for not less than thirty nor more than sixty days; and for the third violation thereof, shall be revoked by the Mayor. For violation of the terms of this chapter or the rules and regulations established and adopted by the Mayor relating to the exhibition or operation of such mechanical or electrically operated amusement device for gambling, such license shall be revoked by the Mayor.
   (b)   Any owner whose application for a license is denied or whose license is suspended or revoked may demand a hearing. A demand for a hearing shall be made in writing to the Mayor.
   The Mayor shall notify such applicant or licensee of such hearing by registered mail directed to the last address of such applicant or licensee on file. In the event that such license is denied, suspended or revoked, the Mayor shall notify such applicant or licensee in the same manner as provided above for notification of hearings.
