503.02 STANDARDS AND PROCEDURES ESTABLISHED.
   An abortion otherwise permitted by law shall be performed only in a hospital or by a licensed physician in a facility which provides the following standards and procedures which shall be met and adhered to:
   (a)   Written verification by two physicians, one of whom may be the physician performing the abortion, of the diagnosis and duration of pregnancy.
   (b)   Recorded preoperative history, including all pertinent records in connection with a referral from a private physician's office, clinic or other abortion facility, and physical examination particularly directed to identification of pre-existing or concurrent illness or drug sensitivities that may have a bearing on the operative procedures or the anesthesia.
   (c)   Laboratory procedures including blood type and RH factor, and those procedures usually required as a prerequisite for prevention of RH sensitization if indicated by blood test.
   (d)   A receiving facility where the patient may be prepared and receive necessary preoperative medication and observation prior to the procedure.
   (e)   Where medical evaluation, examination and referral are made from a private physician's office, clinic or other abortion service, pertinent records as a result of such referral shall be made available at the time the patient is registered and admitted to the abortion service.
   (f)   An operating room properly equipped to conduct safely the abortion procedure and availability on the premises of adequate blood, oxygen, anesthesia, resuscitation equipment and standby ambulance service to handle complications and emergency situations which might occur during the procedure to insure that a patient requiring emergency care at a hospital shall be transported to a hospital within a total transport time of fifteen minutes.
   (g)   Proper anesthesia service administered by a person licensed by the State to administer such medical procedures.
   (h)   A recovery facility staffed with at least one registered nurse in attendance at all times, in which the patient can be observed until she has sufficiently recovered from the procedure and the anesthetic can be safely discharged by the physician, and adequate blood transfusion facilities shall be so available that an emergency transfusion can be given in this facility within thirty minutes.
   (i)   Such facility shall provide preoperative and postoperative psychological counseling by an individual appropriately qualified in the field of psychology.
   (j)   Such facility shall provide the following postoperative care:
      (1)   Patients shall be observed in the abortion service for a reasonable number of hours, not less than three, to ensure that no immediate postoperative complications are present and thereafter such patients may be discharged if their course has been uneventful.
      (2)   Patients in whom any adverse condition exists or in whom a complication is known or suspected to have occurred during or after the performance of the abortion shall remain in the abortion service for further observation or be transported to a hospital.
   (k)   Written postoperative instructions and arrangements for postoperative follow-up examination within one month after the abortion; examination by a certified pathologist of all pathological specimens; and the disposal of human remains in a humanitarian, legal and dignified manner.
   (l)   Adequate permanent records, including the need for administration of antibiotics, blood transfusion, prolongation of patient's stay or hospital admission and other complications. The physician performing the abortion shall keep a record for each abortion containing a pathological diagnosis of estimated age of gestation and height and weight, in grams, of the fetus. This record shall be maintained for inspection by any public official for a period of five years, and would include recording, by code number, the patients who have been aborted and any complications which resulted.
   (m)   The usual informed consent including the operative permit signed by the pregnant woman, and one of her parents or guardian if she is a legal minor.
   (n)   The external identification and promotion of such service shall conform to the provisions of the code of ethics of the national professional organizations of those physicians performing services within such facility.
   (o)   All such services shall permit periodic inspection by the County Department of Health.
   (p)   The service shall meet the standards of the Joint Commission of Accreditation of Hospitals which apply to the areas set forth in subsections (a) through (o) hereof.
