A. All persons required by law or administrative rule to obtain an NPDES general permit #2 from the IDNR are required to obtain a city COSESCO permit. This includes the construction of single- family homes.
B. Applications for COSESCO permits shall be made on forms approved by the city and available at city hall, maintenance annex, and online at the city's website.
C. An applicant applying for a COSESCO permit shall pay fees as follows:
1. An applicant in possession of an NPDES general permit #2 shall pay a fee at the time of application in the amount of one hundred fifty dollars ($150.00). For sites over an acre, there shall be an additional fee of one hundred dollars ($100.00) per acre, or portion thereof. All sites less than one acre will pay a one acre minimum.
2. An applicant applying for a COSESCO permit for a single-family home shall pay a fee at the time of application in the amount of one hundred dollars ($100.00).
3. Consistent with subsection 9-3-4A of this chapter, an applicant in possession of an NPDES general permit #2 and a city COSESCO permit shall pay an annual renewal fee of two hundred dollars ($200.00). Single-family homes are excluded from this requirement.
D. By submission of a COSESCO permit application, the applicant agrees to defend, indemnify and hold the city harmless from any and all claims, damages or suits arising directly or indirectly out of any act or omission by the applicant, or any employee, agent, assign or contractor or subcontractor of the applicant, in connection with the applicant's NPDES general permit #2 and/or COSESCO permit.
