(a) Each applicant for a certificate of registration shall, not less than five nor more than fifteen days prior to the commencement of solicitation furnish the Mayor or his designee with the following information on forms provided by the Municipality.
(1) Name, age and physical description of applicant.
(2) Complete permanent and local address of applicant.
(3) Name and address of the person, firm, corporation or association for whom the solicitation is presently being made and any other person, firm, corporation or association for whom the applicant has solicited during the past three years.
(4) A description of the nature of the business and the goods, services or wares to be sold or otherwise sufficient to identify the subject matter of the soliciting in which the applicant will engage.
(5) The names of all other municipalities in which the applicant has conducted soliciting activities during the past six months.
(6) Whether the applicant has complied with the requirements of Ohio R.C. Chapter 1716 pertaining to charitable solicitations if applicable.
(7) Whether the applicant has ever been denied a license or permit to solicit or had such license or permit revoked, including the time and place of such denial or revocation.
(8) Whether the applicant has ever been convicted of a felony violation or a misdemeanor violation involving moral turpitude including the time and place of such conviction.
(9) The proposed dates and times of the solicitations and the routes to be followed in conducting same.
(10) The make, model, year, color and license plate number of automobiles used by the applicant during the period of solicitation within the Municipality and the number of the applicant's driver's license and state of issuance.
(b) Each applicant shall, prior to receiving a registration certificate, pay registration fees in such amount as provided below:
(1) Peddling from house to house where goods are transported in vehicle - twenty-five dollars ($25.00) per day or one hundred fifty dollars ($150.00) per year.
(2) Peddling from house to house where goods are not transferred in vehicle, twenty dollars ($20.00) per day or one hundred twenty-five dollars ($125.00) per year.
(3) Huckstering in streets, twenty-five dollars ($25.00) per day or one hundred fifty dollars ($150.00) per year.
(4) Selling goods, wares or merchandise from stands or stationary wagon, twenty-five dollars ($25.00) per day or one hundred fifty dollars ($150.00) per year.
(c) Any charitable organization is exempt from paying the registration fee. The term "charitable organization" includes any patriotic, philanthropic, religious, social service, welfare, benevolent, educational, civil or fraternal organization, either actual or purported. The registration certificate for a charitable organization solicitor automatically expires on December 31 of the year of issue.
(d) In addition to the registration fee, each applicant including those associated with charitable organizations, shall reimburse the City for the reasonable cost of producing the registration certificate identification card to be carried by each solicitor. That reasonable cost is set by ordinance.
