(a) Any person desiring a permit required by Section 907.01 shall file with the Director of Public Works a written application for each and every opening, which application shall be signed by the person desiring such permit. Such application shall set forth the purpose for which such opening or excavation is to be made, the extent, size and location of the same and the dates during which such opening or excavation is to be permitted and shall provide that the applicant will faithfully comply with each and every provision contained in this article.
(b) Such application shall also specify the name of the owner of the property served or to be served by the facility located in the street opening.
(c) The provisions of this section shall in no event apply to the repair of a sidewalk by any adjoining property owner. The repair, opening or excavation of any public sidewalk shall be governed by the provisions of Article 909.
