711.03 APPLICATION FOR PERMIT.
   Any person desiring to obtain permission to drill for oil or gas shall, before actual drilling is begun, file a written application, accompanied by a filing and inspection fee of fifty dollars ($50.00) with the Village Recorder at least ten days before such application is acted upon by Council, which applications shall state the following:
   (a)   The name of the applicant, the person for whom such well is being drilled, the name of the driller of the well, the owner of the land upon which the well is to be drilled, and the name of the lessee, if any.
   (b)   The location, as shown upon a blueprint attached to the application, of the well, its proximity to any street, alley, dwelling house or other building.
   (c)   The names of all the owners of the property within a radius of 300 feet of the well location, as shown on the blueprint.
   (d)   The nature and character of the machinery and equipment to be used in the drilling of such well.
   (e)   The streets, alleys, and other ways, if any, necessary to be crossed, traversed, or used in laying pipeline or lines for transportation and marketing such oil or gas.
   (f)   That application has been made to the Department of Energy for the right to drill such well, and that the Department has granted such permit.
   (g)   That the applicant will, if the permit is granted file a bond in the penalty of ten thousand dollars ($10,000) conditioned that it will indemnify and save harmless the Municipality from any and all liability by reason of the drilling, shooting, torpedoing or use of any other explosives in the well, any excavations or other thing, and by reason of any pipeline or lines, and all persons whomsoever by reason of any and all injury to person or to property by reason of the drilling, shooting, use of explosives or otherwise, or by any pipeline or lines built, laid or constructed and/or operated, and,
   (h)   Any additional information that Council may require.
