1107.03 APPLICATION FOR MAJOR SUBDIVISION PRELIMINARY PLAT.
   An application for a major subdivision preliminary plat shall include the following:
   (a)   Application Form. A form, as provided by the Planning and Zoning Director, completed and signed by the applicant and owner of the property. The form shall be submitted with certification of ownership, such as the prior recorded deed reference of the property owner, and, if the applicant is other than the owner, authorization to represent the property owner.
   (b)   Preliminary Plat for Major Subdivision. A major subdivision preliminary plat prepared by a registered surveyor at a scale of not less than one inch equals one hundred feet (1"=100'). The drawing shall not exceed twenty-four inches by thirty-six inches (24"x 36") in size, and shall include the following information:
      (1)   Identification and Location Information:
         A.   Subdivision title, north arrow, scale, date of survey, date of drawing;
         B.   Names and addresses of applicant, owner of the property, development planner and registered surveyor;
         C.   Location map showing the relationship of the proposed subdivision to existing community facilities which serve or influence it.
         D.   Acreage of subdivision; outline and approximate boundary dimensions of subdivision;
         E.   Locations and dimensions of existing roads, utility lines and community features such as schools, parks, airports shall be included.
      (2)   Existing conditions information:
         A.   Boundary lines: the bearings and distance, location and description of all monuments and markers found.
         B.   Easements: the location, width, purpose and restrictions.
         C.   Roads on and adjacent to the tract: the name and right-of-way width and location; type, width, and elevation of surfacing; legally established center line elevations; walks, curb, gutters, culverts, etc.
         D.   Utilities on and adjacent to the tract: the location, size and invert elevation of sanitary storm and combined sewers; location and size of water mains; the location of gas lines, fire hydrants, valves and manholes, electric and telephone poles and street lights. If water mains and sewers are not on or adjacent to the tract, indicate the direction and distance to and size of the nearest ones, showing invert elevation of sewers.
         E.   Ground elevations at intervals as required by the Engineering Construction Standards; benchmarks.
         F.   Subsurface conditions on the tract: any conditions that are not typical such as abandoned mines, gas wells, underground storage tanks, etc.
         G.   Other conditions on the tract: watercourses, marshes, rock outcrops, wooded areas, isolated preservable trees one foot or more in diameter, houses, barns, shacks and other significant features and location of historic sites. A delineation of federally-designated wetlands and floodplains, and of the wetland and riparian setbacks required by the Zoning Code, prepared by a qualified delineator.
         H.   Other conditions on adjacent land: the approximate direction and gradient of ground slope, including any embankments or retaining walls; the character and location of buildings, railroads, power lines, towers and other nearby nonresidential land uses and owners of adjacent unplatted land. For adjacent platted land refer to the subdivision plat by name, date recorded and number.
         I.   Zoning: on and adjacent to the tract.
         J.   Title and certificates: the present tract designation according to official records in the office of the appropriate recorder; the title under which the proposed subdivision is to be recorded, with the names and addresses of owners, notation stating acreage, scale, north arrow, datum, bench marks, certification of the registered civil engineer or surveyor and the date of survey.
         K.   Any portion of bike path projected by the City's bikeway plan for the property being subdivided.
         L.   All oil and/or gas wells on the property being subdivided or within 500 feet of the property being subdivided and including indication of distances of oil and/or gas wells from existing or proposed property lines, existing or proposed roads and existing or proposed habitable buildings.
      (3)    Proposals.
         A.    Roads: the names; right of way and roadway widths; approximate grades and gradients; typical road cross section.
          B.    Other rights of way or easements: the location, width and purpose.
          C.    Locations of proposed utilities.
          D.    Proposed lot lines, lot numbers.
          E.    Location and description of all monuments and markers set or to be set. proposed contours, benchmarks.
         F.    Sites, if any, for any proposed use or structure other than single family dwellings.
         G.   Sites, if any, to be reserved or dedicated for parks, playgrounds or other public uses or money to be donated in lieu thereof.
         H.   Minimum building setback lines conforming to the provisions of the Zoning Code.
         I.   Site data, including total acreage, the number of residential lots, approximate size of each lot and acres in parks, etc.
         J.   Proposed ponds and/or lakes. detention/retention facilities
         K.   An outline of any protective covenants proposed.
         L.   The Planning Commission may require additional preliminary drawings showing information such as road profiles and grades, typical cross sections of proposed roadways, proposed sanitary and storm sewers and water service or the prospective road system of adjacent land owned by the applicant.    
         M.   If the applicant intends to submit final plat applications for platting of the subdivision in phases, the applicant shall submit a written statement describing the intended phases and shall indicate the areas of the phases on the preliminary plat. The Applicant shall further indicate in writing that he is aware of and subject to Section 1105.04(a).
         N.   Drawing for Balance of Subdivided Lot(s). When a proposed major subdivision will result in an unsubdivided remainder area of the original lot(s), the applicant shall submit a sketch plat which demonstrates the intended or potential future use of the remainder area in conformance with the applicable regulations of the City. The intent of this requirement is to ensure that any unsubdivided remainder has the potential for future use in a manner which complies with the plans and regulations of the City.
         O.   Traffic Studies as required by Chapter 1116.
